The Policy Holder Services Bureau examines insurance forms. Its actuaries examine rates, although Montana is one of only a few states without authority to review rates for health insurance. A recently launched new website has online complaint forms at http://www.csi.mt.gov/forms/securities/COMPLAINT%20FORM.pdf.
