The Community and Home Options to Institutional Care for the Elderly and Disabled (CHOICE) provides case management services, assessment, and in-home and community services to individuals who are at least 60 years of age or people of any age who have a disability due to a mental or physical impairment and who are found to be at risk of losing their independence. CHOICE funds may only be utilized after an applicant has been determined and documented ineligible for Medicaid; or, if currently eligible for Medicaid, after a determination that the requested service is not available from Medicaid.

Indiana Family & Social Services Administration

The Family and Social Services Administration (FSSA) was established by the General Assembly in 1991 to consolidate and better integrate the delivery of human services by state government. FSSA is led by the secretary, who is appointed by the governor and is a member of the governor's cabinet. FSSA is a health care and social service funding agency. Indiana Rx Info is a Partnership for Prescription Assistance (PPA) program that connects qualifying, low-income Indiana citizens with discount prescription medicines, direct from the pharmaceutical manufacturers. Indiana Department of Insurance

The Indiana Department of Insurance (IDOI) protects Indiana's insurance consumers by monitoring and regulating the financial strengths and market conduct activities of insurance companies and agents. The IDOI also oversees the administration of several dedicated funds entrusted to the department by the Indiana General Assembly. The department issues reports and consumer alerts so that Indiana consumers are well-informed about the latest concerns in the insurance marketplace. Indiana Department of Insurance — Producer and Agency Licensing Division

The mission of the Producer and Agency Licensing Division is to ensure that producers and agencies are properly educated and licensed to sell insurance in Indiana. As part of its mission, the division approves prelicensing education materials and schools, including approving course content, texts, instructors, and program directors. The division contracts with a test provider to develop, maintain and validate examinations. Examinations are conducted at Ivy Tech College locations throughout the state. Licenses are issued when candidates meet the requirements for licensure. Indiana Department of Insurance - State Health Insurance Assistance Program (SHIP)

The State Health Insurance Assistance Program (SHIP) is a national program that offers counseling and assistance to people with Medicare and to their families. Coordinators provide counseling on Medicare benefits, Medicare Prescription Drug Plans (Part D), Social Security, Medicaid and veterans' benefits. SHIP, formerly the Information, Counseling and Assistance (ICA) grants program, was created under section 4360 of the Omnibus Budget Reconciliation Act of 1990. This act authorized the Centers for Medicare & Medicaid Services (CMS) to make grants to states for health advisory services programs for people with Medicare.

Indiana Family & Social Services Administration, Division of Aging - Indiana Long-Term Care Ombudsman Program

Long-term care ombudsmen are advocates for residents of nursing homes and assisted living facilities. They resolve complaints and educate seniors and long-term care providers about their rights and good care practices. They also provide information to the public on nursing homes and other long-term care facilities and services, residents' rights, and legislative and policy issues. In 1978, an amendment to the federal Older Americans Act required every state to have an ombudsman program that addresses complaints and advocates for improvements in the long-term care system. The ombudsman program is administered by the Administration on Aging (AoA).

The Indiana Association Area Agencies on Aging (IAAAA) were established under the Older Americans Act (OAA) Comprehensive Services Amendments in 1973 to respond to the needs of Americans 60 and older in every local community.
